<p><strong>Alternative Medicine And Health Care In India</strong></p>
<p>Alternative medicine is a major enterprise in India. In November 2009, the government’s Ministry of Health and Family Welfare announced the steps it would be taking to promote ‘Indian Systems of Medicine’ in the country, including spending  Rs. 922 crore on the promotion of AYUSH (Ayurveda, Yoga &amp; naturopathy, Unani, Siddha and Homoeopathy). Major claims have recently been made for alternative medicine – such as that Ayurveda can be used for anaemia or ano-rectal ailments, and that homeopathy can be used to prevent or cure swine flu, or to treat Aids and malaria.</p>
<p> </p>
<p>In an era characterised by evidence-based science and politics, should official endorsement be given to remedies that cannot be scientifically proven? Is alternative medicine a form of quackery, which puts patients at risk? Or is the interest in alternative medicine a positive sign that we are beginning to move beyond a too-narrow approach to disease, towards recognising the more subtle relationship between mind and body and the need to find holistic cures? Do traditional medicines play an important role in preventive medicine, contributing to a more affordable and effective healthcare system than the Western medical model?</p>
<p> </p>
<p><strong>The debate in context:</strong></p>
<p> </p>
<p><strong>What is complementary and alternative medicine (CAM)?</strong></p>
<p>Many discussions about alternative medicine today take place under the umbrella term ‘complementary and alternative medicine’ (CAM). This implies that alternative medicine should not be seen solely as something that can be used instead of conventional medicine, but can and should be used alongside it. CAM refers to treatments that differ from conventional, or ‘allopathic’, medicine – medicine based on scientific testing that is taught to medical professionals. Systems of CAM have often evolved apart from and earlier than the conventional medical approach used in the Western world. Some such systems, such as homeopathy and naturopathy, have developed in Western cultures; others, such as traditional Chinese medicine and Ayurveda, have developed outside of the West. </p>
<p> </p>
<p><strong>Does CAM work – and if so, how?</strong></p>
<p>Critics of CAM argue that the very term is problematic, giving mysticism more scientific credence than it deserves. As the editors of the prestigious Journal of the American Medical Association (JAMA) have argued: ‘there is no alternative medicine. There is only scientifically proven, evidence-based medicine supported by solid data or unproven medicine’. Advocates of CAM, however, argue that the lack of scientific evidence about the efficacy of CAM betrays too narrow an approach to the understanding and treatment of illness. They point to the failure of conventional medicine to cure certain serious diseases, such as cancer, and the extent to which patients suffering from these diseases can be helped by therapies like Ayurveda. They note that conventional medicine has itself moved towards a more holistic appreciation of the treatment of disease, and that some natural remedies or techniques, such as the use of quinine to treat malaria, have been adopted and accepted by practitioners of conventional medicine.</p>
<p> </p>
<p>Even critics of CAM do not dispute that in many cases, alternative remedies can make patients feel better, despite not curing the disease – so isn’t this a worthwhile goal in itself? The importance of preventive medicine, including lifestyle modification, has been globally acknowledged: it is seen as better and more cost-effective to enable people to avoid physical or mental illnesses in the first place, rather than simply trying to treat the disease when it emerges. Alternative medicine, it is argued, has a crucial role to play in this. Furthermore, in a society like India, for which the affordability of healthcare is a major challenge, different models need to be sought that make the most of the country’s experience and expertise, including in the field of alternative medicine. Advocates argue that the orientation of CAM ‘towards self-healing and health promotion (salutogenesis rather than pathogenesis)’ make ‘alternative medicine approaches to chronic diseases especially attractive and affordable for the developing countries’.</p>
<p> </p>
<p><strong>The rise of CAM </strong></p>
<p>One of the key aims of the government scheme the National Rural Health Mission is to ‘encourage a healthy lifestyle and alternative systems of medicine through AYUSH’. In November 2009, Chief Minister, BS Yedyurappa, told a conference that access and awareness of alternative medicines should be spread in rural areas. The officially-recognised status of alternative medicine in India is also indicated by the reach of the Indian Institute of Alternative Medicines, established in 1991 by Dr.Suresh Kumar Agarwal; and the appointment of Smt. S. Jalaja IAS as secretary to the Department of AYUSH.</p>
<p> </p>
<p>The growing interest in, and promotion of, alternative medicine by the political and medical authorities is not confined to India. Despite being at the forefront of advances in conventional, ‘Western’ medicine, the USA provides a significant market for traditionally Indian and Chinese remedies. Organisations such as the National Center for Complementary and Alternative Medicine contribute to the official status of alternative medicine in the USA. In the UK, a major debate took place in 2006 about whether CAM should be provided by the National Health Service (NHS). In the USA and UK, affordability of healthcare is also a key issue, raising questions about whether attachment to the Western model of medicine is appropriate even for Western societies, let alone other cultures.</p>
<p><strong> </strong></p>
<p><strong>A different kind of medicine?</strong></p>
<p>These developments show that CAM is not an unregulated, maverick market, but is fast becoming subject to official and scientific research and regulation. Advocates of CAM argue that quacks can be weeded out by better training and standard-setting. Some also argue that criticism of alternative medicine arises from a Western bias, which distorts the facts about CAM and refuses to countenance its effectiveness. Critics, however, point to concerns about the safety of certain forms of alternative medicine. Discussions about the affordability of alternative compared to allopathic medicine lead to concerns that patients are being duped by irrational treatments that will not make them better. Shouldn’t a universal healthcare system ultimately seek to provide proven cures for illnesses, rather than taking the cheaper but less effective route of alternative medicine?</p>

<p>A). There is an easy solution and compromise to the high cost health care plans in America proposed today.  A low-cost plan that is begun now by allowing those 55 or older to enroll now (if the chose) in Medicare for a nominal price until they reach 65.  By funding only this option today you will be slowing implementing nationwide coverage to all Americans who need it most today.  In later years when this program has been integrated into the system then the age can again be lowered if it is politically wise to do so in the future.  </p>
<p>B). Second, establish a permanent board which places strict price controls on all drugs and services at a fair price that is negotiated out of a position of strength by government officials.  Next, allow all medical plans to seek care outside of their networks if they chose to do so without penalties.  Since strict medical price controls are established by the permanent Medical Price Control Board this makes portability an easy and fair option for all.  And this portability and going outside the insurance networks reestablishes competition back into healthcare in America.  </p>
<p>C).Third allow all alternative practices, therapies, and natural supplements which have been completely excluded from insurance payment and policies to be utilized for any treatments that treats the same dis-eases as current medical practice or for preventive medicine. By allowing substitution of these therapies and treatments this further invites competition to traditional medicine which has had a complete medical monopoly in America for over 60 years.</p>
<p>The FDA and AMA which are staffed by medical doctors who are hostile to alternative medicine in America as competitors should no longer regulate or censure natural therapies which have the safest record of all medical therapies in the world.  Medical doctors who often switch to alternative practices as safer and more effective will no longer be persecuted as traitors and censored by the FDA and local AMA societies.  The FDA should only be allowed to issue general statements or warnings to the general public if they believe a therapy or supplement might be considered harmful.</p>
<p>With this simple low cost ABC healthcare reform in America we can become truly competitive again and natural therapies which often work far better than traditional methods can be utilized by the general public on a wide scale.  &#8212; J.E. Ante</p>

<p>There&#8217;s much debate around the healthcare system reform proposed by President Barack Obama. And while there are many people who protest against it, and those who support it, it is important to know what it is all about in the first place. Here is a short overview of the reform, which is comprised of three essential parts: 1. Assure all American citizens with access to comprehensive and affordable health coverage The main features of this part are: * New Public National Health Plan, which will be very close to the current health coverage provided to federal employees. The main difference is that the new plans will be available to all US citizens for a reasonable price no matter of their financial situation. Deductibles and co-payments will be reduced to minimum, while low-income persons will have the possibility to use additional subsidies. * National Health Insurance Exchange, which will allow US citizens to look for private health plans. It will set regulations on private insurance providers in order to make sure that private plans are not too different form public ones. * New business mandate requiring national enterprises to pay for the Public National Health Plan. * Individual mandate aimed specifically at children. * More support provided to existing programs like Medicaid and SCHIP. 2. Improve the quality of healthcare services and lower their costs This initiative presumes federal financial assistance for improving the quality of the services and lowering the costs, with additional assistance to enterprises that cover high-risk employees. To President&#8217;s belief the following actions may also contribute to lowering cost and improving services: * Special disease management programs for improving chronic care. * Improving transparency in what concerns quality and costs of healthcare offered by providers. * Lowering the rates of medical errors. * Introducing financial incentives to stimulate substantial improvements. * Providing support for researching new and alternative healthcare technologies. * Eliminating ethnic disparities in access and quality of healthcare services. * Popularizing health IT. * Stronger regulation of insurance and drug markets in order to lower medication costs and allow cheap health insurance. * Preventing Medicare private plan participants from overpaying. 3. Wellness and healthy lifestyle promotion This initiative is to be supported through the following actions: * Special wellness programs at working places. * Eliminating child obesity with school activities. * Better education for present and future healthcare workers. * Promotion of healthy lifestyle in communities. Saving possibilities with the new initiatives President Obama estimates that in average a typical American family will be able to save about ,500 on an early basis after the plan will be implemented. These are the possible sources for such savings: * Health IT introduction and implementation. * Improved quality of services. * Limitations on health insurance provider profits. * Federal funding of catastrophic coverage that will lead to cheap health insurance. * Universal coverage availability. An outlay of Rs 775 crore has been allocated under the Tenth Five &#8211; Year Plan. The Plan allocation for 2004-2005 is Rs 181 crore.<br />&#13;</p>
<p>With the opening of the Indian economy, Ayurveda has gained in popularity overseas which is reflected in thronging of Ayurvedic clinics in Kerala and other places by foreigners. Some of the therapies have naturally found a niche in hotels and resorts. As a result, Ayurveda mixed with tourism has become a great source of foreign exchange earnings.</p>

<p>According to CII, India has the potential to attract 1 million tourists per annum, which could contribute up to US $  5 billion to the economy. The Ministry of Tourism is set to tap the great potential of international medical tourists visiting India. Most Indian states have either established themselves as destinations for health care and tourism or are building medical brand images. Add to this, Yogic healing, Transcendental Meditation (TM) along with alternate therapies of ayurveda in India has been repackaged and redefined and goes hand in hand with India’s rise in Health Tourism. According to a CII study &#8220;With yoga, meditation, ayurveda, allopathic, and other systems of medicine, India offers a unique basket of services to an individual that is difficult to match by other countries&#8221;.</p>

<p><strong>Healthcare Bill and Medicare</strong></p>
<p>The Senate Health Bill has been grabbing headlines for months as members of the Senate continue to debate spending, bill allotments, and the merits of improved or universal healthcare coverage. The latest version of this bill in late November and included a price tag of 9 billion, according to the Congressional Budget Office. 0 billion alone would be set aside to help cover doctors&#8217; fees so that they would not suffer from a drastic cut back to reimbursement rates.</p>
<p>Much in keeping with the diversity of American opinion, this bill has come with more than its fair share of controversy, sparking heated debates on both sides of the political aisle.</p>
<p>Bill controversy</p>
<p>Democrats and Republicans are sharply divided over the merits and allocations included in this healthcare bill. The final vote on the bill shows this division; the voting results were nearly entirely along party lines. In the 243 to 183 win by the Democratic-controlled House, only 11 Democrats voted against the bill and only one Republican voted for the bill. That Republican is also an obstetrician &#8211; Rep. Michael Burgess of Texas. Republicans called the 0 billion allocation to cover doctors&#8217; fees a political payoff and labeled it as the Democrats&#8217; way of thanking physicians for their support of President Obama&#8217;s healthcare overhaul.</p>
<p>What&#8217;s included in the bill and how will it affect Americans?</p>
<p>The new healthcare bill, which will go into effect in 2010, is said to provide coverage to 94 percent of Americans. The bill is also estimated to cut the federal deficit by 7 billion during its first ten years. In its second decade, the bill is estimated to cut the federal deficit by as much as 0 billion. Over a 20-year period, it is believed that the bill would cut the federal deficit by 7.</p>
<p>However, in order to cut this deficit, the bill will reduce payments for Medicare plans and reduce spending in a variety of other areas. The Congressional Budget Office estimates that seniors on Medicare will have to pay  billion in higher healthcare premiums over the next 10 years, passing along much of the healthcare expenses to Medicare beneficiaries.</p>
<p>Additionally, because of reduced payments and services covered by the bill, the Congressional Budget Office estimates that Americans will see a 5 billion price increase in doctors&#8217; fees for those doctors who treat Medicare patients. With only a percentage of those fees being covered by Medicare, patients themselves will have to make up the difference out of their pockets. TRICARE (the military healthcare program) beneficiaries would see an increase in fees by about  billion for non-military physicians who see patients enrolled in the TRICARE program.</p>
<p>How can Americans protect themselves from these price increases?</p>
<p>While it may be impossible to predict exactly how this bill will impact Americans and their pocketbooks until the bill is fully approved and implemented, Americans would be wise to begin looking for alternative healthcare solutions. Many Americans on Medicare may want to investigate alternatives to Original Medicare plans, including Medicare Advantage Plans.</p>
<p>Also, as healthcare prices increase, so will the prices of medications and other medical services. Therefore, many Medicare beneficiaries would benefit by enrolling in Medicare Supplement insurance plans to help cover the cost of price increases and of items that are not covered by Original Medicare plans.</p>
<p>Managing the healthcare system has proved to be a challenge and a politically derisive topic &#8211; especially over the past year. While changes are inevitable and are sure to impact every American, Americans can help to protect their health and their pocketbooks by focusing on maintaining or improving their health in controllable ways. Enrolling in affordable healthcare programs, such as a Medicare Advantage or Medicare Supplemental insurance program, may also help to save Americans a significant amount of money at the doctor&#8217;s office.</p>
